Pay Range: $22.77 - $28.07 per hour

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Mental Health Worker will work with adult consumers in the community who suffer from Severe and Persistent Mental Illness. Mental Health Worker provides comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning that incorporates cultural appropriateness.

Qualifications:

Required:

• Bachelor's Degree in mental health field
• Valid NJ Driver's License required

Preferred:

• Bilingual, Spanish preferred, but not required

Scheduling Requirements

• 20 hours/week
• Part Time

Essential Functions:

• Fulfills case management services which include coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, establishing community supports, supportive counseling, crisis intervention, and providing transportation as needed.
• Mental Health Worker will be responsible for the assessment of consumers throughout enrollment in program.
• Mental Health Worker will collaborate with family and health care professionals to develop Individualized Rehabilitation Plans that are consumer centered.
• Mental Health Worker will also works with administrative staff to ensure that all services are documented based on billing and regulatory standards.
